“The provider conversed about my ...”

The provider conversed about my PHI in a hallway amongst other providers who were not my providers. In addition, the provider did not set the proper expectations for me prior to doing a urine drug screen. In addition, the provider uploaded incomplete and inaccurate data in my chart that is causing a delay in my care with a specialist.
