This doctor owns his own part of Gateway Surgical Center and was not willing to do my surgery at Phoenix Baptist Hospital where I have 100% coverage (states he goes to out pt and PBH on website) but instead only wanted me to goto Gateway and pay an out of network $750 copayment. Called offiice after told by ins. about copayment, spoke to surg scheduler who said the doctor said just cancel it then and they never even bother to call me back after accepting me as a pt.
