We do not participate with Medicare, medicaid or HMO. We file for most major ppo as a courtesy to you, with the exception of medicare ppo.
To check if your insurance is in network please contact the office and provide the following information:
Insurance name
Primary member name and DOB
Patient name and DOB if different than primary member and relationship
Member/ subscriber id
Group number
Group or employer name
Insurance phone number
claim address

Payment is expected in full at time services are rendered, for patients in network a pre-treatment estimate will provided in a formal treatment plan and that portion is expected in full at the time services are rendered. For out of network or patients without insurance, payment is expected in full at time of services.

*our office will not enter into a dispute with your insurance over any claim. However, we will provide the necessary documentation your insurance company requests to settle the claim.
