Accepted Insurance

Payment is expected in full for out of network patients , and for in network patients, we will provide a pre-estimated portion on a treatment plan and that amount will be due in full at the time services are rendered.

We file most major ppo Dental insurance as a courtesy , (with the exception of any MEDICARE PPO ) we do not participate with Medicare , Medicaid or HMO's.
Please call our office and provide a valid insurance information including the following information:
Insurance name
Primary member id and DOB
Pt dob if other that primary member and relationship to primary member
group number
group or employer name
claim address
insurance phone number

We will verify your insurance and let you know if we are in or out of network, if you are out of network and have out of network benefits, we can submit the claim as a courtesy to you or provide you with the paperwork to submit to your insurance for re-inbursement.

Our office will not enter into a dispute with your insurance over any claim. However, we will provide the necessary documentation your insurance company request to settle the claim.
