The provider knew the purpose of my appt both before [...] via a portal message with a form she would need to complete and via my conversation with the medical assistant. I am frustrated, therefore, that I am tracking down the necessary documentation to my surgeon that was the sole reason for the appt, and tracking this down on the week after the appt during the provider's absence.
