Summary
University Health Community Care Fairmount provides family-centered medical services for patients of all ages. Our board-certified family medicine providers and nurse practitioners will take care of acute and chronic care needs, and provide preventive and well care services. A diabetic educator is available as well. University Health Community Care Fairmount also offers patients convenient access to behavioral health specialists in the same facility. Providers will arrange for referrals to specialists as needed, and coordinate care through medical specialists affiliated with TMC Lakewood.
