Summary
UF Health's Pediatric Care Coordination Program (PCCP) is a family-centered, community-based model for the care of children with complex medical and social needs.
Patients enrolled in the PCCP have multisystem organ disease, are dependent on medical technology or have a history of frequent hospitalizations. The PCCP was designed to give children with complex medical and social needs more accessible outpatient health management resulting in less ER visits and hospitalizations and more time at home, to heal, grow, and develop.
