“Severe Violation of Medical Ethics”
In 2013, I was a patient of Dr. James Stegeman and his Nurse Practitioner, Teresa Stephens in Springfield Illinois. I just found out on January 4, 2016, that back in 2013, Teresa Stephens placed false information into my medical file. She is now under investigation on why she entered false information into my medical record. She deserves to be fired immediately and never allowed to work in the medical profession ever again. The false information is getting removed from my medical record and I hope she receives the proper consequences of someone in the medical field that places false information into someone's medical records.
