Starting as a focused anticoagulation service, it has since expanded to include a wide range of services to improve medication use and access for our patients. Our clinical pharmacists work closely with you, your provider and your pharmacy to help you with your complex and important medications. They can help manage medication regimens for: anticoagulation, heart failure, high blood pressure, high cholesterol, hepatitis C, long-term opioid use, and weight Loss.
In addition, this service can help patients who take multiple medications and have concerns about managing them or affording their medication.
We also work closely with our specialty pharmacy team to provide extra support and counseling for patients using specialty medications which are often high-cost, complex, and challenging to obtain and use correctly.
