“Petty, vindictive, terrible communication.”
Would not recommend. On my second visit she e-scribed my medications but did not put in a diagnosis code so the insurance refused it. I called Ms. Siuzdak and left at least one message asking for it to be fixed. She did not get back to me at all. After two weeks I got her on the phone. She sent a script to the pharmacy but refused to deal with any prior authorizations for another med. I cancelled my upcoming appointment- she cancelled my scripts at the pharmacy, then informed me by text.
