Patients with Medicare Part B are eligible for an annual wellness visit with a trained Medicare wellness nurse, covered by their plan. Together, you will review your health history, discuss care you are receiving from other providers, review your medications and establish a comprehensive care plan, including recommended screenings and preventative services. You will also learn about advance care planning and advance directives. Call our office to schedule your appointment for this important service.
