I'm pleased that you're asking the question about "Care provider's efforts to include you in decisions about your case." This is a sticking point for me. I will absolutely not engage with a care provider who doesn't show understanding that we're equal partners in mapping out my care.

“My biggest frustration was being...”

My biggest frustration was being told this appointment needed to happen in person to discuss CT results but we didn't have CT results back yet. I appreciated the extra urine sample collection but I wish the full results would've been back before making me come all the way/here/leave work/etc.
