More info: We are unable to accept Independent Health Medicaid or Essential Plans at this time. We do not take Medicare or any Medicare Advantage Plans. All deductibles and co-pays are due at time of service. If you would like to use an insurance that we are out of network with, a superbill will be provided to you so that you can request reimbursement.
