We will happily assist you in completing and submitting your insurance claims. In order for us to do this, you must provide us with properly completed insurance claim forms or current insurance card.
We will accept assignment of benefits by any insurance company if this is allowed by your insurance company. Please be advised that certain insurance companies will not remit payments directly to us (i.e.: Horizon Blue Cross/Blue Shield of NJ).
Submission of insurance and acceptance of assignment of benefits is not a guarantee of payment but a courtesy that we provide. Please note that you are responsible for any unpaid balances not covered by your insurance. It is important for you to know what your individual insurance policy covers, as each policy is different (i.e.: some insurance companies pay for fluoride once per year).
