At Village Endodontics we will file an insurance claim for you if you provide a copy of your insurance card, mailing address, phone number, ID number and group number for your carrier. We will obtain an estimate of the amount your insurance company is expected to pay. You will be responsible for the portion not covered by the insurance company at the time of treatment. The estimate quoted by the insurance companies are occasionally inaccurate due to yearly maximums, treatment not yet processed and limitations on your policy. Your insurance company may also set a procedure limit, which is sometimes lower than reasonable fees for a procedure. Please understand that your insurance coverage is an agreement between you and your insurance company. If there is a difference between our fees and what is ultimately paid by your insurance you will be responsible for the difference.
