In my initial intake from the nurse, she confirmed an allergy that was listed in my chart. When the provider came and saw me, mentioned that same medication as a potential option and asked if I have ever used it before. While it's not that big of a deal, just showed he didn't review my chart prior to coming in as it was documented and confirmed by the nurse.
