“Interestingly, after visit summa...”

Interestingly, after visit summary mentioned us talking about things that never happened. He never said the word "hammer toe" but after visit summary states that is diagnosis. We never discussed me having started wearing new shoes, which I've worn same brand at least 10 years. The AVS makes it sound like we had a long visit, it was less than 10 minutes. Do not understand why he didn't talk to me about issues he wrote about ! A tad annoyed!
