More info: Payment in full is required at time of service. We collect a 20% copay from Medicare patients. Routine eye care plans accepted, however if there is a medical diagnosis we will collect your specialist medical copay and file a claim on your behalf. Deductibles or non covered charges are the responsibility of the patient.
