A significantly decreased libido was my biggest complaint and I honestly thought there was no hope. I figured it was a psychological problem and even considered that I wasn't attracted to my husband any more. More info: Lab work is obtained and an on-line history completed prior to our initial consultation. Insurance coverage for the lab work varies by insurance provider and is discussed at the time of patient calling to for initial lab orders and scheduling of initial consultation. Physician consultations are paid for at the time of the consultation, and the patient is a sales receipt to submit to their insurance for direct reimbursement.

Our office is an "out of network" provider, so we give our patients a sales receipt for services paid for which includes all necessary diagnosis codes and service codes required for the patient to submit to their insurance. Payment is required at the time of service; then the patient submits to his or her insurance for reimbursement.
