Dr. Catherine Kowalski is a participating provider for Cigna, Medicare, Horizon Blue Cross Blue Shield, Empire Blue Cross Blue Shield and all out of state Blue Cross Blue Shields, Oxford, all United Health Care plans except for the Community plan, Operating Engineer’s Local 825,MagnaCare and Qualcare.
We are not participating with Aetna, EXCEPT for Aetna Medicare New Jersey State Employee Retiree plans. The card should have SEHBP Medicare plan printed on the insurance card. We offer affordable payment options for all other Aetna plans.
