Accepted Insurance

We accept most major health insurance carriers. Inclusding:
Aultcare, Aetna, Anthem, Cigna, United Healthcare, Medical Mutual, Ohio Health Choice, etc. (This is not an exhaustive list).
Please call the office to verify that we are contracted with your health insurance carrier. Additionally, we recommend that you check with your insurance carrier regarding what services they will or will not cover. If certain services are not covered, you are responsible for payment at the time of service. We accept Medicare. However, as of August 2013, we are no longer accepting new patients with Medicaid coverage (traditional Medicaid as well as Buckeye, Caresource, and United Health Care Community Plan). Please read our Financial Policy for more detail on all issues related to Insurance & Billing. Additionally, our patient accounts coordinator is available to speak with you from 8 to 12 and 1 to 5 Monday through Friday. You will be considered a self pay patient if you are not covered by any insurance policy or third party payer, if you are covered by insurance plans in which the office does not participate, or if you do not have an insurance card on file with us. Self pay patients are required to bring $125 at the initial appointment and will be asked to make payment arrangements for the balance. Self pay patients will receive a 40% discount across the board for all services rendered.
