“Based off reviewing my chart, sh...”

Based off reviewing my chart, she noticed that my OB/MFM at another clinic had added Type 2 Diabetes to my chart since I was early diagnosed as having GD in my last pregnancy and explained what that does for me as a patient. She also recommended it be amended/removed which I did. She didn't have to that, so thank you to her!
